Medical Assistance
EICAP can provide limited assistance for surgical costs for people that meet the eligibility requirements.
The medical contingency fund was created to pay the medical expenses of qualified patients who are unable to pay their own medical expenses and
- are receiving day surgical services in Bonneville County and
- reside within a 100 mile radius of Idaho Falls, Idaho.
- This fund is to be considered a "last resort" for payment of qualified surgical procedures.
Assistance through this program may not exceed $2,000.
Eligibility Requirements:
- The patient must prove a financial need by completing an application including a Household Financial Statement
- disclosing all assets, liabilities, monthly income, and monthly living expenses; establishing that the patient has no source of payment for any part of the cost of the service or
- the patient has a source of payment for only a portion of the cost of the service.
- The patient has been accepted for day surgical services and has completed all required intake or admissions forms.
- The service meets a degree of medical necessity ranging from life-threatening to general health and comfort.
- Emergency medical services are excluded.
Once an application has been submitted, the following outlines the selection and payment procedure.
- Completed applications are to be delivered to Eastern Idaho Community Action Partnership by the 2nd Monday of each month.
- The Medical Contingency Fund Selection Committee will review the eligible application once a month, assessing the medical priority list, income information and available funds to establish which and how many grants will be awarded.
- Payments will be made directly to the provider in the name of the patient and in the amount determined by the Selection Committee.
