EICAP contracts in-home services provided to older invididuals and their family caregivers who have multiple needs. These services are provided at the direction of the individual (or a family member of the individual). Case managers help to assess the needs of the person and to arrange, coordinate, and monitor an optimum package of services to meet those needs.
The in-home services available through our Case Management are:
- Home Delivered Meals - Nutritious meals provided in the residence of a homebound older person who is unable to independently prepare a nutritious meal and is unable to participate in a congregate meal program. The service must include a nutrition screening and, where appropriate, nutrition education and counseling. Home Delivered Meals provide a mid-day nutritious meal from 1 of 13 Senior Community Centers.
- Homemaker Services - Assistance provided to frail or homebound seniors with housekeeping, laundry, essential errands, and help with meal preparation. Each homemaker is CPR certified. Cost to client is based on a sliding fee scale depending on income.
In addition to in-home services for older individuals, we also provide assistance to their caregivers through our Caregiver Support programs.

The purpose of this screening is to determine whether an older person needs a referral, assistance and client advocacy, or is a potential case management client who should receive a home visit and comprehensive assessment.
Should you become a case management client, our case manager's activities include, but are not limited to: assessment of the individual; development and implemenation of a service plan consisting of both formal and informal resources and services; coordination and monitoring of service delivery; and periodic reassessment.
