Things Needed to Process Loan Application:

  1. 2 years Residence & Employment history
  2. Last year's taxes (2 years if self employed)
  3. 2 most recent bank statements on all accounts
  4. Statements on all loans/debts
  5. Child support payment information
  6. Any state assistance or Social Security info
  7. Child care expenses
  8. Mortgage & Credit report fee $60.00

To apply for Mutual Self-Help Housing, print and complete the following forms.

When you have completed the forms you can:

  • Call Tammy at 522-5391 for an appointment
  • Email them to

Pre-application Form and Photo Release Form require Microsoft Word.
