Long-Term Care Ombudsman

The Long-Term Care Ombudsman provides assistance to residents of long-term care facilities, helps to ensure that residents are aware of their rights, and helps to resolve complaints or concerns as they arise.

The Ombudsman assists with complaints involving residents rights such as:

  • Abuse, gross neglect, and exploitation
  • Access to information
  • Admission, transfer and discharge
  • Autonomy
  • Financial

Resident care:

  • Rehabilitation or maintenance of functioning
  • Restraints

Quality of Life:

  • Activities
  • Dietary issues
  • Environmental issues

Administration:

  • Policies, procedures, attitudes, and resources
  • Staffing

Concerns with outside agencies, systems or individuals:

  • Certification and licensing agency
  • State Medicaid Agency
  • Other systems
  • Complaints other than nursing home or assisted living settings
